For Your First Visit

For your first appointment, be sure to bring:

  1. Signed release form: You will be able to get a records release form from your current doctor’s office. This will allow us to obtain your previous medical records and review your history.
  2. List of medications: Please bring a list of all medications you are currently taking. Also include dosages and how often you take each medicine.
  3. Insurance information
  4. List of questions: Be sure to bring a list of questions/concerns to address with your doctor during your appointment.
  5. Bring a friend: It will be important for you to have moral support during this time as well as someone who can advocate for you as a patient, so try to bring a close friend or family member with you to your appointment.
