Professional Bio
Tara Horr, MD, is an Assistant Professor in the Division of Geriatric Medicine within the Department of Medicine at Vanderbilt University Medical Center. She is an internist who specializes in Geriatrics and Palliative Medicine.
Dr. Horr attended the University of Rochester School of Medicine and Dentistry and completed a residency in internal medicine at Duke University. She subsequently completed fellowships in both geriatric medicine and hospice and palliative care at the Cleveland Clinic. Dr. Horr joined Vanderbilt in 2018 as an Assistant Professor of Medicine within the Division of Geriatrics. She serves as the Outpatient Clinical Services Chief for Geriatrics and oversees the skilled nursing facility program. She cares for patients at The Health Center at Richland Place.
Dr. Horr is passionate about providing care to patients in their homes and serves as the Chief Medical Advisor to Vanderbilt Home Care Services. Dr. Horr helped to launch and oversee the Covid to Home program and served as the Founding Medical Director for Vanderbilt Hospital at Home. She is now the Director of Home-Based Palliative Medicine.
